          Description:

          As part of the clinical case management team, this position supports case management processes to identify, manage, notify patients and physicians as well as potentially apply restrictions at the beneficiary level through claim system. This position works with supervising pharmacists on the team, prepare documentations, collect information on patient cases from both internal and external sources and outreach to physician offices for initiation of cases as well as administrative follow up. Specifically the individual will work to to identify and schedule conferences with appropriate physicians and conduct initial case preparation. In addition, this position will interact with internal matrix organizations including special investigation unit, coverage determinations, appeals as well as care managers for documentation, information retrieval and process coordination. Further, the position will help coordinate any external information inquiries from other Part D plans regarding information on the patient case. This position is also responsible to assist in refining case management business processes for simplification, increased productivity, and compliance to CMS regulations and guidance. In addition, assistance in continued monitoring of new CMS guidance, as well as best practices across the industry will be needed.

          EDUCATION

          The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.

          ADDITIONAL JOB INFORMATION

          This position requires clinical and analytical skills and a recognition of business needs. The candidate must be detail oriented and able to manage multiple case files. Strong communication skills, comfortable interacting with beneficiaries and physician offices.
